Introduction

Normalisation of deviance describes a gradual shift in judgement in which practices that were originally recognised as unsafe, exceptional or outside agreed standards come to be treated as normal because they repeatedly appear to work. The danger is not that people suddenly become reckless. Rather, each successful outcome without an immediate failure encourages the belief that the original safety margin was overly cautious. Projects rarely move directly from safe practice to serious failure. Engineers at NASA and contractor Morton Thiokol had observed erosion and damage to the solid rocket booster O-rings on previous shuttle flights. Several organisational pressures make this mechanism particularly attractive:

Deadline pressure. Teams under delivery pressure naturally ask whether a known issue is “good enough for now”. Repeating this judgement without reassessing cumulative risk gradually lowers standards.

Familiarity. Hazards that are encountered repeatedly become psychologically less alarming. What once prompted escalation becomes another routine item on a meeting agenda.

Performance incentives. If managers are rewarded for maintaining delivery dates rather than exposing uncertainty, evidence supporting continuation receives more attention than evidence supporting delay.

Incremental change. Small adjustments rarely trigger the scrutiny that would accompany a major policy change. Because each individual step appears modest, no single decision feels like crossing an important boundary.

Warning signs that exceptions have become routine

Analytically, the important question is not whether a project contains exceptions—it almost certainly does—but whether the organisation still recognises them as exceptions.

Warning signs include:

  • Teams describing known deviations as “the way we always do it”.
  • Risk acceptance being justified primarily by previous successful outcomes.
  • Temporary fixes remaining in place for months without formal review.
  • Safety or quality thresholds gradually changing without explicit approval.
  • Repeated waivers becoming easier to obtain than resolving the underlying problem.
  • Engineers or specialists expressing concern while management increasingly views those concerns as routine rather than unusual.

Another warning sign is a subtle change in language. Teams stop asking, “Does this meet the intended standard?” and instead ask, “Have we got away with this before?” The first question measures performance against design intent; the second measures it against recent experience.

Preventing deviance drift in project decisions

The practical defence against normalisation of deviance is not to ban every exception but to prevent today’s exception from silently becoming tomorrow’s standard.

Useful practices include:

  • Review repeated waivers together. A single approved exception may be reasonable; ten similar exceptions indicate that the underlying process or design needs attention.
  • Separate schedule decisions from technical judgement. Technical experts should be able to describe risk without simultaneously defending delivery commitments.
  • Record the original rationale. Comparing current practice with the original design assumptions helps reveal how far operational behaviour has drifted.
  • Treat successful near misses as learning opportunities. A problem that caused no visible harm should still be analysed if it required unusual recovery or depended on fortunate circumstances.
  • Invite periodic external review. Fresh reviewers are less likely to accept local assumptions that long-serving teams now regard as normal.

These practices shift attention away from whether the project survived and towards whether its decisions remain consistent with the standards the organisation intended to uphold.

Why this mechanism matters for better thinking

Normalisation of deviance is fundamentally a reasoning error embedded within organisations. It replaces evidence about safe operating limits with evidence about recent survival. The distinction is subtle but crucial.

For anyone trying to improve analytical thinking, the lasting lesson from Challenger is not simply “listen to experts” or “take safety seriously”. It is to recognise that repeated success can sometimes reduce rather than improve judgement. The most dangerous project decisions are often those that no longer feel like decisions at all because accumulated exceptions have quietly become the accepted way of working.
